LSSC requires all incoming student-athletes, returning student-athletes, transfers, and walk-ons to complete medical forms and return them to the Athletic Training Department prior to ANY team-related participation. Click the link below for necessary forms to complete.
In addition to submitting this packet of information- please read the following as general reminders to student-athletes and parents.
- Medical Forms: If you suffered an injury or illness over the summer or the previous school year, you MUST provide us with a copy of any physicians notes or treatment notes relating to said injury/illness. Please include diagnostic test results such as x-rays, EKG, blood work, or MRI/CT scans. If an MRI or CT scan were performed please include a CD copy of the films.
- Insurance: Each year a NEW copy of your insurance card(s) will be required. Please provide an enlarged- clean copy of the front and back of all appropriate cards including your primary health insurance, prescription benefits, dental insurance, and any secondary coverage you may have. Additional information regarding insurance can be located in the policy packet.
- Drug Testing: In accordance with the NJCAA, Lake-Sumter State College will conduct drug testing throughout the year. You may be subjected to testing at any time. Our drug testing policy includes remedial procedures and penalties.
- ADD/ADHD Medications: You should take note of the NJCAA banned stimulants list. If you have been diagnosed with ADD/ADHD by a licensed physician, we require a copy of your results from standardized testing, as well as a physician summary letter and any additional office notes regarding the diagnosis and prescription that is required to treat your condition. Failure to provide the proper documentation could result in a positive drug test.
